What is a Social Care Network?

New York Social Care Networks connect Medicaid members with organizations that screen for health-related social needs and help coordinate appropriate services.

All New York Medicaid members can be screened. Enhanced nutrition, housing, transportation, and other services have additional eligibility requirements.

Who may qualify

You may qualify if you have New York Medicaid.

Medicaid membership allows you to be screened, but it does not guarantee any particular service.

01Can every New York Medicaid member be screened?

Yes. New York Medicaid members in fee-for-service and Medicaid Managed Care can be screened for health-related social needs. Screening does not guarantee an enhanced service.

02Does having Medicaid mean I will qualify?

No. Medicaid membership allows you to be screened. Eligibility for enhanced services depends on individual circumstances, health-related needs, coverage, location, program rules, and approval.

03What nutrition support may be available?

Depending on eligibility and local availability, approved support may include prepared meals, groceries, pantry items, nutrition education, or other food-related services.

04Are approved services available at no cost?

Covered Social Care Network services may be available at no cost to an eligible Medicaid member when the service is approved.

05What happens after I submit the form?

The appropriate team may review the information, contact you for missing details, complete or confirm screening, and explain whether services or other next steps are available. Timing varies.

06Does this website collect medical information?

The screening form asks only for the information you choose to provide for eligibility review and care coordination. It is sent securely to Thrive Care’s referral system. Do not send sensitive information by regular email.

07Can a caregiver help someone apply?

A parent, guardian, caregiver, or authorized representative may help when they have the person’s permission and accurate information.

08Is Thrive Care a government agency?

No. Thrive Care is a service of Lighthouse Care. It is not a government agency and does not determine or guarantee eligibility or approval.
